STS. JOACHIM AND ANN CATHOLIC SCHOOL
REGISTRATION PACKET
2010-2011
Thank you for your interest in Sts. Joachim and Ann School. School hours are from 7:45 a.m. to 3:00 p.m. We have a full-day Kindergarten (children must be 5-years old before August 1st in order to enter Kindergarten) and Grades 1-8.
For registered parishioners, the tuition for one child is $3,868; for two children it is $5,642; and for 3 or more children it is $7, 085.
One $200 non-refundable registration fee is to be returned with the Registration Form. This registration fee covers the 09-10 book, supply and technology fee for one child. If you are registering more than one child, a subsequent payment of $175 per additional child is due by August 1, to cover their related fees. If you prefer, you may include the fee for additional children now.
Each family will be required to generate $200 in profit by using the Shop For Our Schools Program (see info attached), or you may opt to buy out by August 1.
Sts. Joachim and Ann PTO (Parent-Teacher Organization) was formed for the advancement of Catholic education and the welfare of all the children of the school. The PTO Assessment of $75 per family, payable to Sts. Joachim & Ann PTO by a separate check, is due by August 1. Money is spent directly to aid classroom instruction, update the computer lab, classroom equipment/supplies and the library.
To begin the registration process this year, we ask that you follow these steps:
1. Please register in the parish, if you have not already done so. This can be done by
seeing one of the parish priests after any of the weekend Masses.
2. Complete the attached forms.
3. Obtain a copy of the child’s Baptismal Certificate and birth certificate.
4. Provide verification of custody arrangements in cases in which the parents of the
students are divorced (a copy of the portion of the divorce decree, which verifies
custody arrangements, must be provided).
5. Make an appointment (call 636.441.7503) to meet with Father Brockland, pastor of
Sts. Joachim and Ann, and have him sign the Registration Form.
6. Bring the following paperwork to the school office on Sun., January 31, from 9:00 AM to 12 Noon or anytime after this day/time.
(1) Completed Registration Packet
(2) $200 non-refundable Registration Fee
(3) Copy of Baptismal Certificate
(4) Copy of Birth Certificate (official copy – not hospital copy)
(5) Custodial Arrangements (if parents are divorced – a copy of the portion of the divorce decree which verifies custody arrangements)
7. A short assessment is given to each new student. Copies of latest report card and national test scores will be required for all new Grade 1-8 students (see Request for Student Records form attached). The principal, after reviewing the student’s academic records, and in consultation with the parent(s)/guardian(s) and teachers, will recommend placement of the student.
